DATE: Please fill in the date the game was played.

DIVISION: Both the age group and gender are required.

TEAMS: Both team names are needed, with YOUR team listed first.

FINAL SCORE: Pretty much self-explanatory

SCORERS FOR YOUR TEAM: List only goal scorers in this section.
Please try to make certain of the spelling.

SPECIAL MENTIONS: Use this section to list any outstanding plays of the game. Try to mention each player at least once during the season. Good goaltending, great sportsmanship, awesome passing or amazing effort are a few examples.

REFEREEING: This is new this year. We would like to acknowledge our referees when they distinguish themselves during a game. If a ref is particularly patient with young players, calls a great game, or even keeps a cool head during a confrontation, we’d like to know about it. Use your judgement on this one, but remember, many of our referees are just kids themselves, and we would like to encourage them as much as possible.
